Integrated
Family Care

Simply put, Integrated Family Care (IFC) is whole family care. Apex Family Care created Integrated Family Care in 2015. This program was created because we do not believe that children and families should be separated due to poverty or homelessness.

Integrated Family Care is designed to provide holistic care to the family system by preventing the removal of youth from a family home or expediting reunification when a primary barrier is safe and adequate housing and/or acquisition of general parenting skills. IFC is intended to be goal-driven and outcome-based. Apex utilizes formal and informal assessments as well as family input to develop goals to assist the family in reaching identified outcomes.

Our goal is to have families moving in to independent housing three months after beginning the program in a mentor home. We provide aftercare services for three months to ensure stability and long term success for the family.

A key component of IFC is the family mentoring concept. The family receiving assistance is matched with a mentor family. The Mentor provides ongoing hands-on support and guidance to the family 24 hours a day, 7 days a week.

Our Areas of Assistance

A mentor’s role is to be a trusted guide to independence. The mentor welcomes the identified family into their home while teaching and role modeling appropriate parenting and independent living skills. Mentors provide the following supportive services:

Parenting skill development and support


Assistance in the development of independent living skills


Building and maintaining a community support system


Building a supportive and ongoing relationship for the future

Admission Criteria:

●      All participants will be referred by Apex contracting partner.

●      Apex will accept referrals based on availability and appropriate matching.

●      Participants must be homeless or marginally housed.

●      Participants must be willing to voluntarily participate and adhere to the program guidelines and the rules and expectations of the Mentor.

●      Participants must have at least three months of sobriety.

●      Participants must not have any mental health diagnoses which are untreated or may prove to be a barrier to living independently.

●      Participant must be able to become employed or earn an income that allows them to live independently.
